Forms and Policies
Prior to your session we will need you to fill out a client intake form. We have them available in office or you can fill out the PDF below and send to our email at mtmcmassage@gmail.com.
Cancellation Policy
If you are unable to make it you your appointment please contact us at minimum 24 hours before your session. If you cancel same day you may be subject to 50% cancellation fee. No call/no show appointments will be billed 100%. If you have an unpaid invoice you will not be able to come back in until all balances are paid.
If you are new to The Muscle Therapy Massage Center you must fill out our client intake form before your first session. If you can, please print out the form below and bring into your appointment.
